6/12/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Jacaeri was defiant of QMHP-Cs prompts 3-4 times. Jacaeri blamed his dad for his negative behaviors and appeared unapologetic for participating in disruptive actions. Jacaeri was loud and disruptive towards the mental health professional when reviewing his objectives. Jacaeri continued to have a hard time controlling his temper and calming down even when escorted outside of the home by QMHP-C. Behavior: Jacaeri appeared to be a little frustrated at times within the session as he did not like his mother telling QMHP-C about his behaviors and how he does not listen to her. Intervention: QMHP-C initiated a rapport building conversation with Jacaeri in efforts to learn more of his likes and dislikes as well as triggers to negative behaviors listed on his ISP. QMHP-C will review key concepts of being responsible with the family. QMHP-C will educate Jacaeri about being respectful, encouraging, mindful, and fair to peers and authority while making good decisions in the home. QMHP-C will teach Jacaeri necessary coping skills to decrease impulses. QMHP-C will brainstorm strategies with Jacaeri and encourage self-monitoring. QMHP-C will provide mediation and relaxation techniques, as well as, simple praises and encouragement to continue the positive behaviors he was exhibiting. QMHP-C aided this intervention with Jacaeri on social skills. Intervention: QMHP-C introduced Jacaeri to an "Unexpected Behaviors" activity. QMHP-C educated Jacaeri that unexpected behaviors are a behavior that occurs without warning and are behaviors that arenât acceptable in the community like they are at home. QMHP-C presented Jacaeri with some unexpected behaviors with visuals (i.e. get very loud, throw things, say bad words, blame others, donât even try to solve it, keep doing the same thing over and over, and refuse help). QMHP-C informed Jacaeri that displaying unexpected behaviors are disrespectful to your parents, counselors, and peers. QMHP-C probed Jacaeri about certain behaviors and how he sometimes feels like the only way to respond is in a negative manner. QMHP-C offered new ways to heighten Jacaeri awareness of his ill-mannered behaviors, as well as helping him with functioning more constructively within his daily life. QMHP-C advised Jacaeri to make the right choices and to keep up the good work. Response: Jacaeri verbalized that he did not have a good start to his day. Jacaeri verbalized that he was agitated all day but did not give any specific reasons as to why he remained agitated even after his counselor arrived. Jacaeri appeared to grasp a clear understanding of the âunexpected behaviorsâ he often displays in the home especially the behaviors that have affected him the most. Jacaeri was assisted in practicing general communications skills. Jacaeri understood effective problem-solving skills to help solve or overcome a problem or stressor the he is currently facing. Response: Jacaeri participated in reviewing key concepts. Jacaeri showed good understanding of what it means to be respectful, encouraging, mindful, and fair to others. Jacaeri is observed correcting his mistakes and making good choices. Jacaeri was able to utilize coping skills to eliminate stressors and unwanted feelings.
